Stretch marks are caused by rapid expansion of tissue, typically occur in puberty, pregnancy and a burst of obesity. It has historically been a difficult problem to treat; expensive creams, various gadgets have produced little or no results. Fraxel laser SR1500 treatments AND chemical peel treatments have produced visible inprovement. Fraxel laser works by fractionally removing the stretchmarks, penetrating deep into the skin to stimulate collagen to self repair; there is no visible superficial skin injury, therefore, the risk of sacrring and change of skin colour is highly unlikely. It is a well known fact that the off-face skin takes longer to heal therefore the best cosmetic result will take a longer time to achieve.

Stretchmarks bother many people; in my experience, for the first time, Fraxel laser and chemical peels are able to improve the appearance of stretch marks so that people can wear swimming costumes with confidence and it is a big breakthrough in cosmetic laser surgery. The end result is largely dependent on the severity and the quality of the skin and the age of the stretch marks.

Recovery

As there is no visible injury on the surface of the skin there is no dressing required; most stretchmarks are in the concealed parts of the body so there is no interruption to daily life. There is some swelling and redness of the skin in the treated areas which will recover in 7-14 days.

Genital Warts

Genital warts are very common. They are caused by a virus, the human papillomavirus (HPV).

  • There are at least 80 different types of HPV; 20 can infect the genital area.
  • At least 75% of sexually active adults have been infected with at least one type of genital HPV at some time in their life.
  • Most do not develop visible warts; the infection may show up on a cervical smear. This is known as subclinical infection.
  • Visible genital warts are often easy to diagnose by their typical appearance. They are usually due to HPV Types 6 and 11.
  • Some genital warts are often called squamous cell papilloma.

Genital warts may occur in the following sites:

  • Vulva
  • Vagina
  • Cervix
  • Urethra
  • Penis
  • Scrotum
  • Anus

People may confuse normal anatomical structures as warts. These do not require any treatment.

  • Pearly papules (these are in a ring around the glans of the penis )
  • Sebaceous glands on the labia (known as "Fordyce spots")
  • Vestibular papillae (the fronds found in the opening to the vagina)

Transmission of HPV

Visible genital warts and subclinical HPV infection nearly always arise from direct skin to skin contact:

  • Sexual contact. This is the most common way amongst adults.
  • Oral sex. HPV appears to prefer the genital area to the mouth however.
  • Vertical (mother to baby) transmission.
  • Auto (self) inoculation from one site to another.
  • Fomites (i.e. from objects like bath towels). It remains very controversial whether warts can spread this way.

Neonatal infection may arise by passing through an infected birth canal. This can lead to rare complications, such as laryngeal papillomatosis i.e. warts in the throat. Because this complication is unlikely, a caesarean section is rarely indicated simply because a pregnant woman has genital warts.

In small children, genital warts raise the possibility of sexual abuse but in many cases it is due to vertical transmission (see above).

Transmission is common as genital warts often go unnoticed. Subclinical infections can also be infectious.

Often, warts will appear three to six months after infection but latency periods of many months or even years have been reported. Developing genital warts during a long-term relationship does not necessarily imply infidelity.

Visible warts are probably more infectious than subclinical HPV infection. Treating warts seems to decrease the chance of passing on the infection. We cannot tell whether the immune system completely clears the virus from the body, or whether the virus remains hidden but undetectable, capable of re-emerging years later if the immune system weakens. As a result, it is unclear how long someone remains infectious.

The risk of HPV transmission is extremely low if no warts recur a year after successful treatment.

Botulinum toxin

Botulinum toxin A is a very successful medical treatment for facial lines and wrinkles.

How does it work?

BOTOX®, a registered trade mark of Allergan Inc., and Dysport® (Ipsen's product) are different forms of purified botulinum toxin A, and are produced by the bacterium Clostridium botulinum, the cause of a dangerous disease called botulism. Botulinum toxin is a neuromuscular blocking agent, which means it causes paralysis of the injected muscle by preventing the release of acetylcholine from motor nerve terminals. Without its nerve supply, the muscle fibre withers away. The muscle strengthens again as the nerves regenerate.

Botulinum toxin reduces sweating by blocking the sympathetic nerve fibres that control sweat glands.

What can botulinum toxin be used for?

Botulinum toxin treatment was originally introduced to treat muscle spasms, including blepharospasm (spasms of the eyelids), strabismus (squint), cervical dystonia (torticollis of the neck) and spasticity due to cerebral palsy or other muscular diseases.

In patients treated for facial spasms it was noted that facial wrinkling decreased over the treated muscle. This experience lead to the development of botulinum toxin for the treatment of dynamic facial wrinkles.

People often dislike their central scowl lines (called glabellar lines), which are caused by the corrugator and procerus muscles contracting when concentrating, squinting, or frowning. Botulinum toxin injections into these muscles weaken them and successfully reduce the prominence of the line.

Effects of botulinum toxin

Botulinum toxin can also be used to lessen crows' feet, bunny lines, marionette lines and smoker's lines. The treatment can be combined with implantations such as collagen or hyaluronic acid to further improve the appearance. It can also be used to shape the eyebrows into an arch or flare and to correct facial asymmetry.

Botulinum toxin is very successful at reducing excessive localised sweating (hyperhidrosis), especially in the armpits. Multiple superficial injections are given in a grid-like pattern over the affected area. It may also prove helpful to reduce flares in those with pompholyx (a form of hand dermatitis) or Hailey-Hailey disease, when these are provoked by sweating.

How is botulinum toxin administered?

Tiny quantities of the toxin are injected directly into the affected muscles. It takes three to five small injections between the eyebrows to treat the frown line. The injection is almost painless. The treated muscles weaken over the following week or so. Most people do not notice anything. They simply become aware that they are no longer able to contract the frown muscles. They can still lift their eyebrows normally and blink without problems.

You can frown as often as you like in the first day or so, but the treated areas should not be touched. Don't have a facial massage!

To reduce sweating, tiny injections are placed in the affected area at about 1cm intervals; this can be quite painful especially if the hands are injected, so local anaesthetic may be required.

The effect of botulinum toxin starts wearing off within a few weeks but retreatment is not usually needed for three to six months. It can be repeated as required. Many people find after three or four treatments to glabellar lines that they don't need another one for a long time; the muscle has markedly weakened or they have broken the bad habit that led to the frowning or squinting originally.

It is probably the case that the term broken blood vessels is misused most of the time. Frequently, when someone is talking about broken blood vessels, they are referring to visible, enlarged blood vessels on the face. These can occur anywhere on the face, or elsewhere. But the most commonly occur on and around the nose, cheeks, and chin. Off the face, a larger type of enlarged blood vessel occurs which is called the spider vein. Sometimes the term spider vein is used for facial bood vessels as well. The facial blood vessels which are visible are medically termed telangiectasia, or telangiectasias. They are usually medically unimportant, but are frequently a cosmetic concern, because the blood vessels can be quite noticeable. They are easily removed by laser treatment, if desired.

Blue-light therapy. The FDA approved narrow-band, high-intensity blue-light therapy for treating acne. Now widely advertised, this is probably the best-known light therapy for acne treatment. Blue light works by killing the acne-causing bacteria, P. acnes, and is being used to treat inflammatory acne vulgaris that has not responded to other acne therapies. The blue-light products of today do not contain ultraviolet (UV) light, which was a staple of former light therapy used to treat acne. UV light can damage skin and is no longer used to treat acne.

Patients receive blue-light therapy in increments. Generally, eight sessions are given over a four-week period, and each session lasts about 15 minutes. Side effects tend to be mild and include temporary pigment changes, swelling of the treated areas, and dryness. As the results from the following studies show, many patients — but not all — see noticeable improvement with about 55% clearance:

  • 31 patients with symmetrical (same on both sides) facial acne received blue-light treatment on 1 side of their faces. Blue-light therapy was given twice a week for four consecutive weeks. The other side of each patient’s face did not receive any treatment so that researchers could judge the effectiveness of the blue-light therapy. The researchers concluded that blue-light therapy is an effective acne treatment unless the patient has nodulocystic acne lesions, which tend to worsen when treated with blue light. (

  • 25 patients with inflammatory acne on their faces had half of their face treated with blue-light therapy (8 sessions over 4 weeks) and the other half with clindamycin (a topical antimicrobial shown successful in treating acne). Patients were instructed to apply the clindamycin twice a day for four weeks. After four weeks, the clindamycin side showed a 22.25% improvement, and the side treated with blue light improved by 39%. However, after eight weeks and without further treatment, the side treated with clindamycin was better able to maintain results. (United States)2

  • 10, 13, and 25 patients were enrolled in three separate studies designed to learn more about the effects of blue-light therapy on papulo-pustular (inflammatory) acne. In all three studies, more than 80% of the patients treated with blue-light therapy responded to the treatment. The patients who responded saw a 59% to 67% reduction in inflammatory acne lesions. (Israel)3

  • 30 patients with mild to moderate acne were treated twice a week with blue-light therapy for up to five weeks. After five weeks, acne lesions decreased by 64%. However, not all patients saw improvement; 20% remained unchanged or experienced a worsening of their acne.

As you can see, the number of patients involved in these studies is small. Clinical trials conducted to test new drugs generally must enroll hundreds to thousands of participants before submitting the data to the FDA for consideration. With such small numbers, statistically valid conclusions cannot be drawn. Research also is needed to learn the long-term effects. At present, blue-light therapy appears to improve acne vulgaris in some individuals. This therapy appears most effective for inflammatory acne lesions and may not be suitable for patients with nodulocystic acne.

The oral antibiotics most commonly prescribed for acne include:

  • Tetracycline – tetracycline (Panmycin®), minocycline (Mino-tabs®, Minomycin®), doxycycline (Doxine®, Doxy® & Doryx®), limecycline (Tetralysal®). These are not suitable for children younger than 10 years old because they may stain teeth yellow.
  • Erythromycin - Emu-V®, Eryc®, Eromycin®, E-mycin®, EES® & ERA®
  • Trimethroprim - Triprim® & TMP®
  • Cotrimoxasole - Apo-Sulfatrim®, Trisul®, Trimel®, Bactrim® & Septrin®

Side effects and risks (see also, DermNet's page on tetracycline)

  • Allergy - antibiotics can cause a variety of rashes in those susceptible. These can be mild or life-threateningly severe. Allergy to tetracycline or erythromycin is very uncommon but arises in more than 2% of those on trimethoprim or cotrimoxasole. Tell your doctor if you have ever reacted badly to an antibiotic.
  • Photosensitivity may be a problem for those taking doxycycline. Taking the medicine after the evening meal reduces the risk of sunburn.
  • Gastrointestinal disturbance affects about 5% and includes nausea, colicky pain and diarrhoea.
  • Thrush (candidiasis) affects 5% of treated women and is most often vaginal. Thrush can affect the oral mucosa or body folds (intertrigo). It is less likely with erythromycin than with tetracycline.
  • Bacterial resistance - most frequently arises with intermittent use of topical antibiotics. To reduce this, apply topical antibiotics liberally twice daily or combine with benzoyl peroxide or topical retinoids.
  • Possible increased chance of failure of the oral contraceptive pill. The risk is very small.
